Transcript Requests and Enrollment Verification
The Midstate College Student Records Department is here to help with your transcript requests, enrollment verifications, and degree verifications. Transcript requests are processed in-house; the National Student Clearinghouse processes enrollment and degree verifications.
A fee of ten dollars for each official transcript ordered is payable in advance with credit card, check, cash, or money order. Credit card payments can be made over the phone by calling 309-692-4092. Students who have outstanding financial obligations to the College will not be issued an official transcript until the debt is cleared by the Accounting Department; however, unofficial transcripts may be issued at no cost. Requests are typically fulfilled in one to three business days.
Please complete and return the Transcript Request Form.
